DOWNLOAD

1. Click on the form you need and save it to your computer or device
2. Download and install Adobe Reader from here:

FILL-OUT

1. Open the saved form on your computer with Adobe Reader
2. Fill out the EMPLOYEE required fields (To highlight the areas to fill-out click “Highlight Existing Fields” in the top right corner)

SIGN

1. Click on “Sign” in the upper right menu
2. Click on “Place Signature” and in the pull-down menu select “Draw my signature”
3. Using your mouse or finger sign in the box
4. Place signature on “Signed” line and resize if necessary
5. Click “Send Signed Document”
6. Click “Confirm and Send”
7. Save the file to your computer
8. In the next window select “CANCEL” (This will keep from uploading and sending your document through Adobe EchoSign)
9. Quit Adobe Reader

UPLOAD

1. In the form below, fill-out “Your Name”, “Your Email” and the “Form Name”
2. Click “Choose File” and select the saved signed form file from your computer
3. Click “Send”
